\n We're recruiting for an Adult Nurse Case Manager to join a leading specialist complex care provider delivering high-quality care to adults with complex healthcare needs in their own homes across North West London.\n

The Role\n
\nAs an Adult Nurse Case Manager, you will:\n
\n

\n
• Manage a caseload of adults with complex healthcare needs living within the community
\n
• Complete comprehensive clinical assessments and regular care reviews
\n
• Develop and review person-centred care plans
\n
• Undertake risk assessments and clinical governance activities
\n
• Complete competency assessments and clinical supervision for Healthcare Assistants
\n
• Support the mobilisation of new care packages
\n
• Liaise with families, GPs, Consultants, Commissioners and multidisciplinary teams
\n
• Respond to clinical concerns and provide expert clinical guidance
\n
• Support CQC compliance and maintain high clinical standards across your caseload
\n

What We're Looking For\n

\n
• Registered Adult Nurse (RGN/RN Adult) with a valid NMC PIN
\n
• Experience supporting adults with complex healthcare needs
\n
• Clinical skills in one or more of the following:\n
  \n
  • Tracheostomy care
  \n
  • Ventilation
  \n
  • PEG feeding
  \n
  • Spinal injuries
  \n
  • Neurological conditions
  \n
  • Complex respiratory care
  \n
\n
• Experience within community nursing, complex care, case management, district nursing, ICU, HDU or acute hospital settings
\n
• Excellent communication and clinical decision-making skills
\n
• Full UK driving licence and access to a vehicle
\n
