Adults Social Worker- CHC [Trafford]

Our Client, a Greater Manchester council is seeking an Adult’s Social Worker to join their CHC Team.

Responsibilities of this role:

\n
• Provide social work support to adults across Trafford.
\n
• Complete strength-based assessments in line with the Care Act 2014.
\n
• Develop tailored, person-centred support plans.
\n
• Conduct reviews of care and support arrangements.
\n
• Promote independence and empower adults to maximise their strengths.
\n
• Recognise the potential for change and support positive outcomes.
\n
• Apply the legislative framework for Adult Social Care in practice.
\n
• Work proactively with a wide range of adults with care and support needs.
\n
• Respond effectively to safeguarding enquiries in accordance with the Care Act 2014 and local policies and procedures.
\n
• Work both independently and as part of a team.
\n
• Collaborate within a Multi-Disciplinary Team (MDT) environment, including Continuing Healthcare (CHC) professionals.
\n
• Apply knowledge of the Mental Capacity Act 2005 in practice.
\n
• Professionally challenge decisions where necessary to ensure the best outcomes for residents.
\n
• Maintain effective communication with service users, families, colleagues, and partner agencies.
\n

Requirements of this role:

\n
• Must be a Qualified Social Worker, registered with Social Work England.
\n
• Must have minimum of 3 years post qualifying social work experience
\n
