Director of Care Management, Nurse Practitioner (Pediatric/PICU)
Job Title: Director of Care Management, Nurse Practitioner
Job Type: Full-Time (37.5 hours/week)
Schedule: Monday-Friday | Day Shift (Generally 8:00 AM - 5:00 PM)
Location: Valhalla, NY
Key Responsibilities
Lead, mentor, and manage the inpatient RN Case Management team.
Develop and implement departmental goals, policies, and best practices.
Oversee patient admissions, referrals, care coordination, and discharge planning.
Evaluate incoming referrals to determine clinical appropriateness and rehabilitation needs.
Collaborate with physicians, nursing leadership, social work, patient access, and other interdisciplinary teams to optimize patient flow.
Identify opportunities for quality improvement and implement initiatives to improve operational efficiency.
Analyze referral trends, patient length of stay, utilization data, and discharge barriers to enhance patient care and organizational performance.
Build and maintain relationships with referring healthcare organizations and providers.
Assist in developing strategies to improve patient throughput while maintaining high standards of clinical care.
Participate in departmental planning, performance improvement initiatives, and leadership meetings.
Ensure compliance with regulatory standards and organizational policies.
Required Qualifications
Master of Science in Nursing (MSN).
Active Nurse Practitioner license (or ability to obtain licensure in the hiring state before employment).
Previous experience working in a pediatric healthcare setting.
Prior Pediatric Intensive Care Unit (PICU) experience.
Strong leadership, communication, and interdisciplinary collaboration skills.
Experience coordinating patient care across the continuum.
Preferred Qualifications
Previous leadership or management experience in Case Management or Care Coordination.
Experience with electronic medical record (EMR) systems.
Quality Improvement or Performance Improvement project experience.
PRI and Screen Certification is a plus.
