Our organization is on the lookout for full-time physicians specializing in adult and senior care to enhance our expanding post-acute care team in the Southern (Hanover) area. Become a part of our team and contribute significantly to delivering exceptional, compassionate care to our elderly population. The post-acute care team provides services across various skilled nursing facilities throughout the region.

We encourage applications from primary care physicians, hospitalists, and those with geriatric training.

Position Overview:

  • Deliver direct patient care across multiple settings, including skilled nursing, rehabilitation, personal care/assisted living, independent living, and clinics.
  • Engage with facility leadership, hospital discharge teams, care management, and other specialties to ensure high-quality care aligned with shared objectives.
  • Experience a diverse range of healthcare fields in post-acute care, from general primary care and internal medicine to geriatrics and acute rehabilitation services. Our clinicians collaborate with various specialists, enhancing their knowledge and skills.
  • Work alongside and supervise Advanced Practice Providers.
  • Utilize EPIC EMR, online diagnostics, including PACS imaging and lab results.
  • Participate in clinical rounds Monday through Friday, with appealing daily schedules and flexible models. Overnight and weekend on-call duties are limited to phone coverage, shared fairly among partners and APPs, with favorable ratios.

Professional Qualifications:

  • Board certified or board eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
  • A team player capable of delivering personalized patient care and demonstrating positive clinical outcomes.
  • Dedicated to providing high-quality, compassionate care to patients.
  • Eligible for licensure in Pennsylvania.
