Work Dates Needed: August 3 for a 3 month contract Preferred Schedule: Fortuna Monday - Thursday 4-10's 8am - 6pm \n
\nWorksite Setting: FQHC, 1 other APP on site & 1 RN - all ages \n
\nScope of Work: up to 24 scheduled - Family Practice all ages \n
\nLicenses, Certifications, Requirements: CA, DEA, must have Medicare (PAVE), ORP (order, refer, prescribe), Partnership PHC (Medi-cal) minimum requirement \n
\nEMR: Ochin Epic \n
\nPay Rate Preferred: $100/hour.
