Per Diem Physician - Post Acute & Senior Care Services (Lititz)

Our organization is on the lookout for Per Diem physicians specializing in adult and senior care to enhance our expanding post-acute care team in the Lancaster/Lititz area. This role is vital in delivering exceptional, compassionate care to our elderly patients. The post-acute care team operates across various skilled nursing facilities throughout the county.

We encourage applications from primary care physicians, hospitalists, and those with geriatric training.

• Deliver direct patient care across a range of facilities, including skilled nursing, rehabilitation, personal care/assisted living, independent living, and clinics.
• Engage with facility leadership, hospital discharge teams, care management, and other specialties to ensure high-quality care aligned with shared objectives.
• Explore diverse healthcare fields within post-acute care, from general primary care and internal medicine to geriatrics and acute rehabilitation services, enhancing your expertise through collaboration with various specialists.
• Work alongside and supervise Advanced Practice Providers.
• Utilize EPIC EMR, along with online diagnostics such as PACS imaging and lab results.
• Participate in clinical rounds Monday through Friday, with appealing daily schedules and flexible models. Overnight and weekend on-call duties are limited to phone coverage, shared equitably among partners and APPs, ensuring favorable ratios.

• Board certified or board eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
