Overview:
\n Our specialized nephrology practice is seeking a Nurse Practitioner to provide high-quality care for patients with chronic kidney disease. New graduates are welcomed to apply. Key Responsibilities:

\n
• Evaluate and manage patients with chronic kidney disease
\n
• Conduct patient rounds and monitor treatment progress
\n
• Perform comprehensive patient assessments and medical histories
\n
• Develop and implement treatment plans in collaboration with physicians
\n
• Provide patient and family education on disease management
\n
• Coordinate care with interdisciplinary healthcare teams
\n
• Maintain accurate and timely medical documentation
\n
• Support continuity of care and positive patient outcomes
\n

Qualifications:

\n
• Master's or Doctoral degree from an accredited Nurse Practitioner program; active California NP license required
\n
• National NP certification (e.g., FNP, AGNP) preferred
\n
• Strong clinical, communication, and patient care skills, with the ability to work effectively within a multidisciplinary team
\n
