As a Home Health Physical Therapist, your key responsibilities will include:
- Delivering home-based physical therapy to patients who have experienced a decline in mobility, strength, and functionality due to illness or injury
- Collaborating with a home health team, which may consist of an RN, LPN, CNA, HHA, OT, and SLP, to focus on the patient’s care plan and objectives
- Monitoring movements, performing assessments, documenting progress, and adjusting care plans as necessary
- Guiding patients through exercises and hands-on therapy to manage pain and enhance mobility
- Educating patients about their care plans, established goals, exercises, and self-care techniques
- Coaching family members on the care plan, therapy objectives, and safe exercise assistance
Essential qualifications for Home Health Physical Therapists include:
- Graduation from an accredited Physical Therapist Program and possession of an active PT license in California
- A minimum of 1 year of physical therapy experience, preferably in home healthcare settings
- Current CPR certification
- Familiarity with state and federal home health regulations
- Strong clinical judgment, excellent interpersonal skills, dexterity, and compassion
