Our organization is looking for a full-time Primary Care Nurse Practitioner (NP) or Physician Assistant (PA) to become a valuable member of our clinic team in Jacksonville, FL.

As a Nurse Practitioner (NP) or Physician Assistant (PA) at our facility, you will collaborate with a diverse group of professionals, including physicians, pharmacists, care coaches, and behavioral health specialists, all dedicated to providing personalized, high-quality care.

You will report directly to the Associate Medical Director and work at our clinic located on Phillips Hwy, FL.

Your Responsibilities
  • Deliver comprehensive care to senior patients in accordance with clinical standards.
  • Work closely with your care team to ensure coordinated, high-quality service.
  • Oversee referrals, hospital/SNF coordination, DME, and home health services.
  • Document patient care with assistance from quality-focused coders.
  • Participate in on-call rotations and contribute to growth initiatives at your center.
  • Have experience in providing value-based care.
  • Work on-site at the clinic.
What We Are Seeking

Essential Qualifications:

  • Master’s degree in nursing with board certification.
  • Active, unrestricted NP or PA license in Florida.
  • Experience in value-based care, primary care, or Medicare Advantage environments.
  • Possession of an active DEA license and Medicare/Medicaid provider numbers, or the ability to obtain them.
  • Familiarity with HEDIS quality indicators and Medicare guidelines.

40 hours

Compensation Range

The salary range provided below reflects a good faith estimate of the starting base pay for full-time (40 hours per week) employment at the time of posting. Actual pay may vary based on geographic location and individual qualifications, including skills, experience, education, and certifications.

$123,800 - $170,300 per year

This position is eligible for a bonus incentive plan based on individual and/or company performance.
