School-Based Physical Therapist - Ilfeld, New Mexico
Job Description

This contract opportunity near Ilfeld, NM allows you to utilize your specialized skills in a supportive educational environment. You’ll work one day per week, providing essential physical therapy services to a caseload of approximately eight students, fostering their growth, mobility, and participation in school activities.
Key Qualifications & Experience:
- Valid Physical Therapy license, with experience in a school setting preferred
- Familiarity with PT evaluation, intervention, and documentation processes for students ages Pre-K through high school
- Must have a valid Medicaid NCI number
- Proven ability to collaborate with special education teams, teachers, and families
- Strong communication and organization skills
- Commitment to compliance with FERPA, IDEA, and state regulations
- Must pass a comprehensive background check
Role Responsibilities:
- Deliver individualized and group physical therapy services to students based on IEP goals
- Conduct assessments and contribute to eligibility, progress, and review meetings
- Develop treatment plans aimed at maximizing students’ independence, safety, and participation within the school environment
- Document treatment sessions and progress in accordance with district and Medicaid requirements
- Provide training and consultation to school staff and families to ensure continuity of care
- Partner with multidisciplinary teams to facilitate optimal outcomes for each student
