Position: Home Health Physical TherapistIn this role, you will:
- Design and implement prescribed physical therapy treatments for patients in their homes, aiming for the highest possible level of physical function.
- Evaluate and document patients' physical capabilities to assist in physician assessments and inform treatment objectives.
- Create and execute a rehabilitation program aligned with the physician's treatment plan and the goals set by the patient care team.
- Provide clear written care instructions to Physical Therapy Assistants (PTAs) and Home Health Aides, supervising their performance and documentation.
- Record patient observations, interventions, and evaluations in a timely and comprehensive manner.
- Communicate patient progress to the physician, Clinical Manager, care team, patient, and family, while submitting all necessary evaluations, treatment plans, progress notes, and discharge summaries.
- Degree from an accredited Physical Therapy Program recognized by the APTA.
- Current and unrestricted Physical Therapy license.
- Valid CPR certification.
- Experience working collaboratively with a healthcare team.
- At least one year of experience as a Physical Therapist.
- Pay per visit/unit: $59.00 - $83.00
- Annual base salary: $93,000 - $128,000
Tuberculosis Screening: This position involves direct patient interaction and is part of our Tuberculosis (TB) screening program. If selected, you will need to undergo TB screening.
Driving Requirements: This role is included in our driver safety program, necessitating a valid state driver’s license and personal vehicle liability insurance. You must maintain vehicle insurance that meets or exceeds your state’s minimum requirements, or at least $25,000 for bodily injury per person, $25,000 for bodily injury per event, and $10,000 for property damage, whichever is higher.

Compensation RangeThe salary range provided reflects a good faith estimate for full-time (40 hours per week) employment at the time of posting. Actual pay may vary based on geographic location and individual qualifications, including skills, experience, education, and certifications.
