Key Responsibilities

• Conduct initial and ongoing assessments of patients’ conditions.
• Develop, modify, and evaluate care plans tailored to identified issues, involving patients and their families in the process.
• Implement therapy interventions using standard physical therapy techniques appropriate for each patient’s condition.
• Educate patients, families, and staff by assessing educational needs.
• Ensure continuity of care by participating in discharge planning, providing home instructions, recommending equipment or modifications, and suggesting follow-up care as necessary.
• Supervise the activities of physical therapy assistants, students, and other support staff.
• Maintain accurate documentation of patient responses to therapy and the necessity for skilled interventions as required by regulatory standards.
• Work in a consistently alert and safe manner.
• Perform additional duties as assigned.

Qualifications

• Education: A Bachelor’s, Master’s, or Doctorate degree in Physical Therapy or a related field, as required for state licensure.
• Experience: No prior experience is necessary.

Physical Demands

• Regularly required to stand and frequently lift, carry, push, or pull objects weighing up to 25 lbs.
• Frequent communication through hearing and speech.
• Visual acuity for both near and far distances.
• Regular walking, reaching, gripping, and data entry tasks.
• Occasional bending, climbing, kneeling, and lifting patients or objects weighing between 25-50 lbs.
• Occasional driving and rare crawling may be required.

Licensure and Certifications

• Basic Life Support Healthcare Provider (BLS HCP) certification is required within 90 days of employment. Additional resuscitation certifications may be necessary based on departmental needs.
• Must hold a valid Physical Therapist license in the respective state of practice (Illinois, Missouri, Oklahoma, or Wisconsin).

Work Schedule

This position is a day shift role, with a commitment of 32 hours per week.
