A clinical pharmacist specializing in home infusion plays a vital role in preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications for patients within their homes. Serving as the central point of a multidisciplinary healthcare team, this position requires oversight of sterile compounding, medication dose adjustments, and delivering critical education to both patients and caregivers.

Key Responsibilities:

  • Prepare specialty pharmaceuticals and IV solutions in a sterile environment, adhering to all state and federal regulations.
  • Monitor patient lab values and calculate pharmacokinetic dosages, including vancomycin and Total Parenteral Nutrition (TPN) adjustments.
  • Provide detailed counseling on specialty medications and treatment plans to patients and caregivers.
  • Educate patients and caregivers on infusion pump usage, sterile techniques, and catheter care in a home setting.
  • Collaborate with prescribing physicians, home health nurses, and multiple internal teams to ensure coordinated care and reduce hospital readmissions.
  • Maintain compliance with all applicable regulatory standards including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA.
  • Manage pharmacy inventory, including refrigerated items, and handle packaging and shipping to ensure timely delivery to patient homes.

Desired Experience and Qualifications:

  • Minimum two years of infusion specialty pharmacy experience
  • Familiarity with IVIg therapy is advantageous
  • Understanding of specialty therapy payer requirements, reimbursement processes, and contract obligations
  • Current and good standing NY pharmacist license
  • Proficiency with the CareTend system
