Position Summary

\n

The Hospital at Home Physician provides acute-level inpatient care to medically appropriate patients in their homes through a hybrid care model that includes in-home visits by nursing staff, virtual rounding, and interdisciplinary coordination. This physician will lead care delivery in a value-based, patient-centered model designed to reduce inpatient utilization while

\n

maintaining high clinical quality and safety standards. This role combines traditional hospitalist-level acuity with innovative care delivery, leveraging remote monitoring, mobile diagnostics, and interdisciplinary home-based services.

As a physician in this innovative model, you will:

    Manage acute medical conditions in a home-based setting

    • \n
  • \n

    Lead interdisciplinary care teams including RNs, APPs, case managers, and virtual support staff

    • \n
  • \n

    Deliver high-quality, value-based care with strong clinical outcomes

    • \n
  • \n

    Improve patient satisfaction and reduce unnecessary hospital stays

    • \n

Clinical Scope

\n

Typical diagnoses may include:

\n
    \n
  • \n

    CHF exacerbation

    • \n
  • \n

    COPD exacerbation

    • \n
  • \n

    Pneumonia

    • \n
  • \n

    Cellulitis

    • \n
  • \n

    UTI

    • \n
  • \n

    Post-surgical recovery

    • \n
  • \n

    Other DRG-appropriate conditions per program guidelines

Who We're Looking For

    Physicians Board Certified/Board Eligible in Internal Medicine or Family Medicine

    • \n
  • \n

    Experience in hospital medicine or acute care required

    • \n
  • \n
