Perform essential pharmacy services within a 35-bed Long-Term Acute Care Hospital (LTACH) specializing in critical illness recovery. This role focuses on supporting medication distribution, sterile product preparation, medication reconciliation, clinical pharmacy interventions, patient counseling, drug therapy monitoring, and ensuring regulatory compliance.

Responsibilities:

  • Conduct thorough medication reconciliation, developing precise prior-to-admission medication lists by gathering information from patients, families, and healthcare sources.
  • Provide discharge counseling to patients and families regarding their medications.
  • Monitor drug therapy for contraindications, interactions, allergies, and appropriate dosing.
  • Offer clinical consultation and medication recommendations to physicians and healthcare practitioners.
  • Participate in antimicrobial stewardship activities, including interpreting culture results and making infection-related therapy recommendations.
  • Perform pharmacokinetic and anticoagulation dosing and monitoring.
  • Maintain medication inventory, prepare and dispense sterile and non-sterile products per standards.
  • Verify medication orders and oversee pharmacy support personnel.
  • Manage controlled substances, emergency medications, floor stock, and medication expiration reviews.
  • Ensure compliance with federal and state laws, JCAHO/accreditation standards, and facility policies.
  • Engage in staff education, orientation, and competency programs, providing resources and fostering interdepartmental communication.
  • Uphold safety policies, identify hazards, and promote a secure patient-care environment.

Qualifications / Desired Experience:

  • Active pharmacist license relevant to assignment.
  • Hospital or inpatient pharmacy experience preferred.
  • Proficiency with EPIC electronic health records.
  • Strong clinical skills in medication reconciliation, antimicrobial stewardship, pharmacokinetics, and anticoagulation.
  • Excellent communication skills and ability to work independently in a LTACH/hospital setting.
  • Experience with discharge medication lists and patient counseling.
