Position Overview:- Deliver direct patient care across multiple facilities, including skilled nursing, rehabilitation, personal care/assisted living, independent living, and clinics.
- Engage with facility leadership, hospital discharge teams, care management, and other specialties to ensure optimal quality care with aligned objectives.
- Explore a diverse range of healthcare fields, from general primary care and internal medicine to geriatrics and acute rehabilitation services. Our clinicians collaborate with various specialists, enhancing their knowledge and skill sets.
- Work collaboratively with and supervise Advanced Practice Providers.
- Utilize EPIC EMR, online diagnostics, including PACS imaging and lab results.
- Participate in clinical rounding Monday through Friday, with attractive daily schedules and flexible models. Overnight and weekend on-call duties consist solely of phone coverage, shared equitably among partners and APPs, with favorable ratios.

- Board certified or board eligible in Primary Care, Internal Medicine, Family Medicine, or Geriatric Medicine.
