collaborative, team-based practice

currently staffed by:

1 full-time physician 2 advanced practice providers (APPs) 1 part-time physician

Outpatient-only primary care

with a focus on preventive medicine, chronic disease management, and continuity of care Flexible patient panel: adult-focused Internal Medicine or

Family Medicine (ages 13+) Utilize
