Job Title: CAPP Health Educator

Reports to: CAPP Health Educator Supervisor
Salary Range: $22.16 - $22.75 hourly
Position Summary:
The CAPP Health Educator delivers evidence-based sexual and reproductive health education to adolescents in schools and community settings throughout ACR Health's service area. This position focuses on pregnancy prevention, STI/HIV prevention, healthy relationships, and positive youth development through classroom education, community outreach, individual risk-reduction counseling, and collaboration with community partners.
The ideal candidate is an engaging educator who is passionate about empowering youth, promoting health equity, and creating safe, inclusive learning environments.
Primary Responsibilities
" Deliver evidence-based sexual and reproductive health education to youth in schools and community settings.
" Facilitate classroom presentations, workshops, and community education events.
" Provide individualized and group risk-reduction counseling for youth ages 11-21.
" Connect youth to family planning, HIV, STI, HCV, and other supportive community services.
" Develop culturally responsive educational materials and presentations.
" Conduct community outreach promoting healthy relationships, consent, teen dating violence prevention, and LGBTQ+ inclusion.
" Recruit, develop, and facilitate the CAPP Youth Advisory Board and youth leadership activities.
" Build and maintain collaborative relationships with schools, community organizations, and youth-serving agencies.
" Maintain accurate, timely, and confidential client documentation, progress notes, encounters, referrals, and required reports.
" Assist with community needs assessments and monitor trends affecting adolescent health.
" Participate in community coalitions, staff meetings, trainings, outreach events, and agency initiatives.
" Complete required curriculum and professional development training.
" Maintain confidentiality in accordance with HIPAA, Article 27-F, and ACR Health policies.
" Represent ACR Health professionally in the community.
" Perform other duties as assigned.
Authority and Independence
The CAPP Health Educator works under general supervision of the CAPP Health E while exercising independent judgment in planning educational activities, conducting outreach, maintaining confidential records, and providing youth-centered services within established agency policies and procedures.
Minimum Qualifications
Education and Experience
" Bachelor's degree in health education, Public Health, Human Services, Education, Social Work, or related field.
" Experience working with adolescents in educational, health education, prevention, counseling or community-based settings.
" Experience serving diverse populations and communities.
Knowledge, Skills and Abilities
" Knowledge of adolescent sexual and reproductive health, pregnancy prevention, HIV/STI prevention, contraception, and healthy relations.
" Strong public speaking, presentation, and group facilitation skills.
" Ability to establish rapport with youth from diverse backgrounds.
" Excellent communication, organizational, documentation, and time management skills.
" Proficiency with Microsoft Office Suite, including Outlook, Word, Excel, PowerPoint, Teams, and SharePoint.
" Commitment to maintaining confidentiality and protecting client information in accordance with HIPAA and Article 27-F.

Other Requirements
" Ability to work a flexible schedule, including occasional evenings and weekends.
" Valid New York State driver's license.
" Reliable access to a vehicle with current insurance for work-related travel, community outreach, training, and occasional client transport (ownership of the vehicle is not required but consistent access is necessary).

FLSA Status: Non-Exempt
Location: Syracuse Office Hours per week: 35 Work Schedule: Full Time in Office, occasional evenings and weekends may be required for meetings, workshops, outreach, and events.
