Job Opportunity: Registered Nurse - Hospice Homecare

Our organization is in search of a dedicated and empathetic Registered Nurse to join our Hospice Homecare team. This role focuses on delivering exceptional, patient-centered care for individuals at the end of their lives within the comfort of their own homes.

Key Responsibilities

• Oversee symptom management and provide appropriate medications and treatments.
• Collaborate with an interdisciplinary team to create personalized care plans.
• Educate and support both patients and their families throughout the care process.
• Ensure a safe and effective care environment in the home setting.
• Document all patient interactions and care details in the electronic health record (EHR).
• Participate in on-call rotations as required.

Qualifications and Requirements

• Current Registered Nurse license.
• Experience in hospice or palliative care is preferred.
• Strong communication and interpersonal skills.
• Ability to work collaboratively within a team.
• Compassionate approach to patient care.

Working Conditions

This position is primarily field-based, requiring travel to patients' homes. Flexibility in scheduling may be necessary to accommodate patient needs.
