Clinical Infusion Pharmacist

We're hiring a Clinical Infusion Pharmacist for a full time direct hire opportunity in Syracuse, NY. If you have recent home infusion experience and enjoy providing direct clinical support while collaborating with multidisciplinary healthcare teams, this is an excellent opportunity to join a growing specialty pharmacy.

Position Details

• Direct Hire
• Full time, Monday-Friday
• Schedule: 8:00 AM - 4:30 PM
• On call rotation required

Responsibilities

• Prepare and verify sterile compounded IV medications and specialty infusion therapies.
• Monitor patient therapies, including pharmacokinetic dosing, TPN management, and laboratory values.
• Provide clinical interventions and medication recommendations to optimize patient outcomes.
• Educate patients and caregivers on infusion medications, infusion pumps, sterile technique, and catheter care.
• Collaborate with physicians, nurses, intake, reimbursement, and pharmacy operations teams to ensure seamless patient care.
• Maintain compliance with all applicable state and federal pharmacy regulations and accreditation standards.
• Manage specialty medication inventory, including refrigerated products, and oversee preparation for safe home delivery.
• Utilize CareTend pharmacy software.

Requirements

• Active New York Pharmacist License in good standing.
• Minimum 2 years of recent home infusion or specialty infusion pharmacy experience.
• Experience with IVIg therapy.
• Strong knowledge of sterile compounding, specialty pharmacy operations, and clinical monitoring.
• Experience with specialty therapy payer requirements, reimbursement processes, and contract obligations.
• Experience using CareTend preferred.
• Strong communication and interdisciplinary collaboration skills.
