Your Responsibilities
As a Home Health RN Case Manager, your duties will include:
- Conducting skilled nursing visits in patients' homes
- Performing assessments, initial care visits, and developing care plans
- Collaborating with the interdisciplinary care team
- Overseeing LPNs and Home Health Aides
- Documenting care using an iPad with AI assistance (OASIS experience is a plus)
- Educating patients and their families to foster independence
- Ensuring the continuity, quality, and safety of patient care
What We Expect From You
- Active, unrestricted Registered Nurse (RN) license
- Valid driver's license and dependable transportation
- CPR certification
- At least 1 year of RN experience (preferably in home health or acute care)
Preferred Qualifications
- Experience working with aging or adult patient populations
- Highly organized with excellent time-management skills; capable of multitasking, prioritizing a busy caseload, and proficient with technology and electronic documentation systems
