Saratoga Hospital Geriatric Medicine seeks a compassionate and dedicated Geriatrician to join our established IHI Age-Friendly Health System and Geriatric Medicine team to help launch the Saratoga Hospital Fellowship in Geriatric Medicine. This physician will serve as a core faculty member of the Fellowship in addition to clinical duties, with protected academic time. The primary clinical setting is an outpatient consultative geriatric assessment clinic and will play a key role in shaping, growing, and anchoring the hospitals Center for Successful Aging initiative.

Key Responsibilities:

  • Supervise fellows and family medicine residents, modeling evidence-based geriatric practices.
  • \n
  • Provide didactic lectures and case-conference teaching.
  • \n
  • Assist in developing and refining the geriatric curriculum.
  • \n
  • Participate in resident/fellow milestone assessments and serve on the Clinical Competency Committee.
  • \n
  • Provide comprehensive geriatric assessments and consultative care for older adults with complex medical and social needs, offering expertise in geriatric syndromes, polypharmacy management, functional assessment, dementia care, and advance care planning. Work with other interdisciplinary team members including rehabilitation staff, case management, social work, and other providers in a team approach to providing value-based care for the elderly.

  • Collaborate closely with the hospitals existing inpatient consultative geriatric service to ensure smooth transitions and continuity of care across settings.

Qualifications:

    \n
  • MD or DO with board certification in Geriatric Medicine (Internal Medicine or Family Medicine trained).
  • \n
  • Eligible for licensure in New York State.
  • \n
  • Strong clinical skills in geriatric medicine, including diagnosis, management, communication with patients and families, and care planning for older adults.
  • \n
  • Commitment to value-based, patient-centered care models with a consultative focus.
  • \n
  • Excellent communication and interpersonal skills with patients, families, and healthcare professionals.
  • \n
  • Interest in program development and innovation in Geriatric Medicine.
  • \n
