Position Overview
The Care Manager Assistant - LPN plays a vital role in facilitating the coordination of patient care and services. This position emphasizes high-quality care through a collaborative approach that involves coordinating, monitoring, and assessing services based on the needs and interactions with patients, all while adhering to relevant laws, regulations, and organizational standards.
Education, Experience, and Licensure Requirements- A nursing diploma (either an associate's or bachelor's degree in nursing).
- Current licensure/registration in the relevant state, with an active LPN license in good standing required throughout employment.
- Preferred: At least one (1) year of clinical experience in a post-acute care environment.
- Preferred: Experience in case management, utilization review, and discharge planning.
- Ability to produce written communication that is clear, concise, and well-structured.
- Strong organizational skills with the capability to prioritize tasks and manage time effectively.
- Contribute to the organization's objectives and support strategic plans.
- Possess expert knowledge in the relevant field of practice.
- Make independent decisions when necessary.
- Familiarity with practices, procedures, and regulations governing operations in a post-acute care facility.
- Implement and interpret departmental programs, goals, policies, and procedures.
- Demonstrate proficiency in all competency areas, including general management duties, patient rights, and safety protocols.
- Maintain professional relationships with colleagues, vendors, and others.
- Ensure confidentiality of all proprietary and sensitive information.
- Understand and adhere to organizational policies, including those related to harassment and compliance.
- Exhibit integrity and professionalism by following the organization's Code of Conduct and completing required compliance training.
- Efficiently utilize relevant computer systems, including data entry and retrieval.
- Act as a liaison between external case managers, families, physicians, and community resources.
- Provide training and education to patients, families, colleagues, and other care providers.
- Implement standards of practice for care management, ethical performance, and care coordination.
- Deliver excellent customer service and maintain a positive attitude towards patients.
- Assist in the safe evacuation of patients when necessary.
- Demonstrate reliable and consistent attendance.
- Utilize reasoning skills and sound judgment in various situations.
- Communicate effectively and collaborate within an interdisciplinary team.
- Engage in physical activities such as sitting, standing, bending, lifting, pushing, pulling, stooping, walking, reaching, and moving intermittently during work hours.
- Read, write, speak, and comprehend the English language proficiently.
