Speech Language Pathologist (SLP) – Home Health Services

Available Positions: Full-Time, Part-Time, or Per Diem

Service Areas: Duval/Clay and St. John's Counties

JOB PURPOSE:

The Speech Language Pathologist will deliver evaluative, consultative, and therapeutic interventions to home health patients referred by licensed physicians in their residences. Effective communication with Home Health partners, United Rehab partners, patients, their families, and other relevant stakeholders is essential.

KEY RESPONSIBILITIES:
  • Conduct evaluations, assessments, and treatments in line with policies and individualized care plans, adhering to established timelines.
  • Collaborate with the interdisciplinary team and attending physician to ensure continuity of care, integrating and managing the patient care plan, and making referrals to other disciplines as necessary.
  • Exhibit knowledge of home care practices and demonstrate competency in specific patient care skills required for effective service delivery.
  • Identify and respond to patient care priorities with initiative and sound judgment.
  • Assist in training and orienting new staff members.
  • Utilize approved educational materials for patient and family education.
  • Report any suspected violations to supervisors until resolved.
  • Adhere to the corporate compliance program.
  • Meet agency productivity standards while using time and resources efficiently.
  • Participate in center/agency surveys, mandatory in-services, case conferences, staff meetings, and programs such as the Performance Improvement Program as required.
  • Follow established safety protocols and report any job-related hazards as necessary.
  • Maintain current licensure, certification, and clinical records to comply with federal, state, and agency regulations.
  • Demonstrate the ability to be self-directed, prioritize tasks, manage time effectively, and achieve outcomes and strategic goals by set deadlines.

LICENSURE, CERTIFICATION, AND EDUCATION REQUIREMENTS:
  • A valid professional license in the state of practice is required.
  • Graduation from an ASHA-accredited program is necessary. For foreign-trained candidates, the program must be recognized as substantially equivalent to U.S. entry-level education in Speech Language Pathology by an ASHA-approved credential evaluation organization.
PREFERRED SKILLS AND EXPERIENCE:
  • Experience in Home Health is preferred.
