Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

A home infusion clinical pharmacist plays a critical role in preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications to support patient care in the home setting. Acting as the central hub within a multidisciplinary team, this position ensures sterile compounding practices, optimizes medication dosing, and delivers essential education to patients and caregivers.

Responsibilities:

• Prepare specialty pharmaceuticals and IV solutions under sterile conditions, adhering to state and federal regulations.
• Assess laboratory results, calculate pharmacokinetic dosing (e.g., for vancomycin), and monitor Total Parenteral Nutrition (TPN) to adjust patient treatments as needed.
• Provide thorough patient counseling and education on specialty medications, infusion pump usage, sterile techniques, and catheter maintenance.
• Collaborate with prescribing physicians, home health nurses, and various support teams (intake, referral, reimbursement, nursing, clinical, and pharmacy operations) to ensure coordinated therapy management and prevent hospital readmissions.
• Maintain regulatory compliance with State Board of Pharmacy guidelines, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other applicable standards.
• Manage pharmacy inventory, including refrigerated products, and oversee the packing and shipment of medications and supplies for safe delivery to patients’ homes.

Qualifications:

• Current and valid New York pharmacist license in good standing.
• Minimum of 2 years’ experience in infusion specialty pharmacy.
• Experience with IVIg therapy preferred.
• Strong knowledge of specialty therapy payer requirements, reimbursement processes, and contract obligations.
• Familiarity with CareTend system is a plus.
