This opportunity is for a PRN Pharmacist providing multi-site coverage in the Cleveland and Greater Cleveland, OH area. This contract role spans 13 weeks with an ASAP start date and focuses on day-shift availability on a PRN/as-needed basis. The candidate will work across several hospital and rehabilitation pharmacy settings, requiring travel between assigned facilities.

Key qualifications include an active Ohio Pharmacist license and hospital/inpatient pharmacy experience is strongly preferred. Candidates must demonstrate strong clinical and communication skills, proficiency with EPIC software (preferred), and the ability to work independently. Experience in medication reconciliation, discharge medication management, antimicrobial stewardship, pharmacokinetic and anticoagulation dosing/monitoring is essential.

Responsibilities encompass completing accurate medication reconciliation and prior-to-admission medication lists by collecting information from multiple sources. Providing discharge medication counseling to patients and families is a critical function. The role also involves monitoring drug therapy for contraindications, interactions, allergies, and appropriate medication selection, as well as offering clinical consultation and recommendations to healthcare providers. Active participation in antimicrobial stewardship activities and proper medication-use review supports safe and effective patient care.

Pharmacy operations duties include maintaining medication inventory, preparing and dispensing medications—including parenteral—and ensuring controlled substance accountability. The pharmacist will manage emergency medications, monitor storage conditions, and uphold compliance with all federal, state, and facility pharmacy regulations.

This role also contributes to education and staff development by participating in training, offering medication education to healthcare professionals and patients, and promoting positive interdepartmental collaboration and quality improvement initiatives.
