The organisation delivers person-centred support tailored to individual needs, working closely with families, commissioners, and professionals to ensure safe, high-quality placements and sustainable support solutions.About the RoleThe Assessment & Referrals Manager is responsible for overseeing the end-to-end referral and assessment process across our services in Northwest England. The role ensures timely, high-quality, and person-centred assessments, leading to appropriate placements that meet individual needs while supporting organisational growth, occupancy, and compliance.You will act as a key link between commissioners, families, and internal teams, ensuring seamless transitions into services and building strong external relationships to maintain a consistent pipeline of referrals.Key ResponsibilitiesAct as the main point of contact for incoming referrals from local authorities, ICBs, case management companies, and other stakeholdersManage and coordinate the referral process from initial enquiry through to placementSupport managers in conducting person-centred assessments covering needs, risks, compatibility, and support requirementsProduce assessment reports, recommendations, and costings where requiredEnsure referrals meet eligibility criteria and service suitability requirementsBuild and maintain strong relationships with commissioners, local authorities, social workers, care managers, families, and external professionalsSupport occupancy growth by converting referrals into placementsWork closely with Operations, Housing, Finance, and Quality teams to ensure service readinessMaintain accurate referral tracking systems and produce reports on referral activity and outcomesMonitor performance against referral conversion rates, occupancy levels, and assessment turnaround timesProvide performance updates to the Group Relationship ManagerEnsure assessments comply with CQC standards, the Care Act 2014, commissioning requirements, GDPR, and organisational policiesKey Skills and QualificationsEssential:Proven experience in supported living / health & social care referrals and assessmentsStrong understanding of Adult Social Care frameworks and Commissioning and Funding processesExperience conducting complex needs assessments and placement matchingExcellent communication skillsAbility to manage multiple referrals in a fast-paced environmentDesirable:Experience in business development / occupancy growthKnowledge of learning disabilities, autism, and/or mental health servicesRelevant qualification in Health & Social Care or related fieldBenefitsCompetitive salary packageFull-time permanent positionOpportunity to work with a growing and respected supported living providerCollaborative team environmentOngoing professional development opportunitiesMeaningful role supporting positive outcomes for individuals accessing servicesHow to apply:If you have the skills and experience required for this position, click “apply” today and check your inbox for an email providing more information on how to complete your application and provide any other supporting documents.You must be authorised to work in the UK.
