\nLee Health is seeking a Skilled Nursing Physician!
\nLee Health is a nonprofit organization with more than 17,000 employees, 4,500 volunteers, and more than 1,200 employed primary care and specialty care providers with multiple practice locations throughout Southwest Florida. The organization has four acute care hospitals and one specialty hospital that together house over 1,800 hospital beds.
\nPosition Information:
\nWe have a team of 5 physician SNFists and 5 APP's as well as looking to grow to provide services for 3 post-acute rehabilitation facilities. That in itself is an incredible patient to provider ratio and an example of Lee Health's commitment to provide a Quality Based care approach! As a SNFist, you will dedicate your expertise to post-acute care environments. This role will allow you to address the specific needs of patients transitioning from hospital care to rehabilitation or long-term residency in SNFs (Skilled Nursing Facility) and SNU's Skilled Nursing Unit). You will create a pivotal shift towards specialized, quality care tailored for a vulnerable segment of the population often in dire need of attentive, personalized medical oversight. You will also complete comprehensive medication reconciliation management and treatment plans of care to ultimately have a goal to return our patients back to their original home setting as quickly as possible.
