Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

A clinical pharmacist specializing in home infusion plays a critical role in preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications for patients in their homes. This position is located in Columbus, OH, and offers a direct hire opportunity.

Key Responsibilities:

  • Sterile Compounding: Accurately prepare specialty pharmaceuticals and IV solutions within a sterile, ventilated environment while adhering to all state and federal regulations.
  • Clinical Monitoring: Evaluate laboratory values, perform pharmacokinetic dosing calculations (such as for vancomycin), and monitor Total Parenteral Nutrition (TPN) therapy. Provide thorough patient counseling and education on specialty medications and treatment protocols. Monitor drug therapies to identify potential therapeutic problems and recommend clinical interventions.
  • Patient Onboarding & Education: Teach patients and caregivers proper use of infusion pumps, sterile techniques, and catheter maintenance to ensure safe home administration.
  • Care Coordination: Work closely with prescribing physicians, home health nurses, and multiple internal teams including intake, referral services, reimbursement, nursing, clinical services, and pharmacy operations to deliver integrated patient care and prevent hospital readmissions.
  • Regulatory Compliance: Ensure strict adherence to regulations from the State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other applicable standards.
  • Inventory & Shipping: Oversee management of pharmacy inventory, including refrigerated products, and prepare medications and supplies for safe, efficient delivery to patients.

Qualifications:

  • Minimum of 2 years’ experience in infusion specialty pharmacy
  • IVIg experience is highly desirable
  • Familiarity with specialty therapy payer requirements, reimbursement processes, and contract obligations
