Primary Care Physician

Our organization is in search of a part-time physician specializing in Family Medicine, Internal Medicine, or Geriatric Medicine. This role involves providing services at Mercy LIFE Assisi House in Aston, PA, and Mercy Life Valley View in Elwyn, both situated in the greater Philadelphia area.

Under the guidance of the medical director, the Primary Care Physician (PCP) will deliver essential medical care to participants of the PACE Organization (PO). Responsibilities include conducting assessments, collaborating with the interdisciplinary team to develop care plans, providing direct medical care to participants, and evaluating the effectiveness of these care plans. The physician will work collaboratively with other healthcare professionals and make independent decisions regarding medical needs and health regimens, as well as perform necessary medical procedures.

Position Overview:

Mercy LIFE Assisi House serves as the retirement convent for the Sisters of St. Francis of Philadelphia, located directly across from the Our Lady of Angels Convent, the congregation's motherhouse. The facility adopts a person-centered care approach, fostering a familial atmosphere within its 125-bed, multi-level care environment, which includes specialized units for Alzheimer's and dementia care.

Mercy Life Valley View offers both housing and the Mercy LIFE PACE program tailored for senior adults, specifically catering to a community of deaf seniors. Candidates must be open to learning American Sign Language.

This role entails working two 8-hour days at Assisi House and one 4-hour day at Valley View, with flexible scheduling during the week.

No night, weekend, on-call, or holiday coverage is required for this position!

Key Responsibilities:

• Engage as a member of the interdisciplinary team (IDT) and participate in initial, semi-annual, unscheduled, and annual assessments. Report any changes in participants' baseline status to the appropriate staff daily.
• Conduct initial and ongoing assessments of participants, develop care plans, and implement these plans. Ensure the primary care treatment plan aligns with the overall care plan established by the interdisciplinary team.
• Refer participants to medical specialists as necessary. Manage the care of participants in the nursing home, conducting regular visits in collaboration with the IDT, and maintain communication with nursing home staff for continuous monitoring of PACE participants on a case-by-case basis. Admit nursing home participants to the hospital when required.
• Oversee and manage participants' interactions with medical specialists and inpatient care, ensuring effective communication and follow-up regarding visit outcomes.
• Review medical outcomes to identify best practices and enhance participant health. Complete documentation accurately and utilize appropriate diagnostic coding.
• Advise the Medical Director on strategies to improve accountability of PO services to participants and referral sources, keeping the Medical Director informed about necessary material and human resources as the program grows.
• Assist in developing policies and procedures, standards of care, and conduct ongoing monitoring and evaluation of patient care practices and service delivery. Provide guidance and training to staff on medical and quality assurance matters.
• Participate in the provider on-call rotation as needed.

Minimum Qualifications:

• Doctorate Degree (MD or DO) required, along with a current and active plenary physician license to practice in the state of PO. CDS Certification and DEA Registration are mandatory.
• Current BLS certification is required.
• Board Certification in Internal Medicine or Family Practice is required; Board Certification in Geriatrics is preferred.
• Willingness to learn American Sign Language is essential.
• A minimum of one (1) year of experience working with the elderly or frail population is required.
• A valid driver's license, proof of insurance, and access to a personal vehicle are necessary.
• Strong communication, problem-solving, and organizational skills are essential. Ability to effectively communicate with diverse groups is required.
• Proficiency in Microsoft Office suite is necessary.
• Ability to adapt to changing priorities is important.
• Critical thinking skills and the ability to prioritize workload are essential.
• Occasional travel within the Center's geographic area may be required.
