Our organization, located in Spartanburg, SC, is in search of a full-time Board Certified/Board Eligible Internal Medicine or Family Medicine physician, or a fellowship-trained Geriatrician, to join the Medical Group of the Carolinas - Senior Health team as a Skilled Nursing Facility Physician (SNFist). This role involves delivering clinical care to inpatients at a skilled nursing facility, which includes providing direct medical attention, on-call support, and emergency care for geriatric residents. The SNFist will collaborate with interdisciplinary care teams and act as a liaison with families regarding care decisions for residents.

Key Highlights of the Position:
  • Flexible work schedule
  • On-call rotation of 1:3 weeks, including weekends (phone consultations only), with Nurse Practitioners as the first point of contact
  • Independence in managing high-acuity patients requiring complex medical care
  • Facilitate and participate in daily Care Team Meetings at the assigned facility
  • Monitor progress on care plans and identify early warning signs for potential readmissions during the critical post-discharge phase
  • Engage in discussions about care goals, transition planning, and advanced directives with patients and their families as needed
  • Act as the attending physician for patients transitioning to Hospice care, with a focus on symptom management at the end of life
  • Supervise Nurse Practitioners providing care in Skilled Nursing Facilities
  • Participate in Quality Improvement, Peer Review, and Care Management Meetings as requested
