Job Description
This opportunity is for a licensed Ohio Pharmacist to provide PRN support across multiple hospital and rehabilitation sites in the Cleveland area. The position requires flexibility for day shifts and the ability to travel between assigned locations. This 13-week contract role focuses on inpatient pharmacy services, clinical consultation, and medication management within fast-paced hospital environments.
Key Responsibilities:
- Perform comprehensive medication reconciliation, ensuring accurate prior-to-admission medication lists.
- Deliver discharge medication counseling to patients and their families.
- Monitor patient drug therapies for contraindications, interactions, allergies, and appropriate dosing.
- Provide clinical recommendations to physicians and healthcare team members.
- Engage in antimicrobial stewardship, including interpretation of culture results and therapy optimization.
- Conduct pharmacokinetic and anticoagulation dosing and monitoring.
- Support medication-use reviews, quality assurance, and Pharmacy & Therapeutics committee activities.
- Manage medication inventory, prepare and verify medication orders, including STAT orders.
- Maintain compliance with federal, state, and facility pharmacy regulations and documentation.
- Participate in staff education, orientation, and serve as a resource within the pharmacy team.
- Promote workplace safety, patient confidentiality, and a culture of continuous quality improvement.
Desired Experience and Qualifications:
- Active Ohio Pharmacist license is required.
- Hospital or inpatient pharmacy experience strongly preferred.
- Availability for PRN/on-call day shifts with flexible scheduling.
- Willingness to travel between multiple Cleveland-area healthcare facilities.
- Proficiency with EPIC electronic medical records is preferred.
- Strong clinical judgment, communication skills, and ability to work independently.
- Experience in medication reconciliation, antimicrobial stewardship, pharmacokinetics, and anticoagulation management.
