Job Description
Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.
A home infusion clinical pharmacist plays a critical role in preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications for patients receiving care at home. Serving as the central hub in a multidisciplinary team, this pharmacist oversees sterile compounding, adjusts medication dosages, and provides essential education to patients and caregivers to promote safe and effective therapy management.
Responsibilities:
- Sterile Compounding: Prepare specialty pharmaceuticals and IV solutions within sterile, ventilated environments compliant with state and federal regulations.
- Clinical Monitoring: Evaluate lab values, calculate pharmacokinetic dosing (e.g., vancomycin), and monitor Total Parenteral Nutrition (TPN), adjusting treatments accordingly. Provide thorough patient counseling regarding specialty medications and treatment plans. Identify potential therapeutic issues and suggest clinical interventions when needed.
- Patient Onboarding & Education: Instruct patients and caregivers on the use of infusion pumps, sterile techniques, and catheter care to ensure therapy safety at home.
- Care Coordination: Work closely with prescribing physicians, home health nurses, intake, referral, reimbursement, nursing, clinical services, and pharmacy operations teams to facilitate cohesive care plans and reduce hospital readmissions.
- Regulatory Compliance: Ensure adherence to State Board of Pharmacy regulations, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other relevant standards.
- Inventory & Shipping: Oversee pharmacy inventory management, including refrigerated products, and prepare medications and supplies for timely and secure delivery to patients.
Qualifications:
- Minimum of 2 years of experience in infusion specialty pharmacy.
- IVIg experience is a plus.
- Familiarity with specialty therapy payer requirements, reimbursement processes, and contract obligations.
- Current and good standing NY pharmacist license.
- Experience with CareTend system preferred.
