Richboro Rehabilitation & Nursing Center is seeking a dedicated Registered Dietitian to join our team and make a difference in the lives of our residents.

Daily Responsibilities:

\n
• Complete required nutrition assessments per MDS schedule
\n
• Develop and implement individualized nutrition care plans
\n
• Monitor and revise nutrition interventions for desired outcomes
\n
• Provide nutrition recommendations addressing identified diagnoses
\n
• Support compliance with PDPM principles
\n

Required Skills:

\n
• Experience with MDS, PDPM, and NFPE
\n
• Bachelor’s degree in Nutrition and Dietetics
\n
• Active Registered Dietitian (RD/RDN) credential
\n
• Proficiency in PointClickCare or similar EMR preferred
\n
• Long-term care experience strongly preferred
\n
