Home Infusion Clinical Pharmacist - Liverpool, New York
Job Description
Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.
A home infusion clinical pharmacist opportunity is available in Syracuse, NY, offering a direct hire placement with a Monday to Friday 8 AM to 5 PM schedule. Candidates will participate in an on-call rotation, typically handling fewer than five after-hours calls per week.
Key Responsibilities:
- Prepare specialty pharmaceuticals and IV solutions in a sterile, ventilated environment, following all relevant state and federal regulations.
- Assess laboratory values, perform pharmacokinetic dosing calculations (e.g., vancomycin), and monitor Total Parenteral Nutrition (TPN) for treatment adjustments.
- Provide detailed patient counseling and education related to specialty medications and treatment plans.
- Educate patients and caregivers on infusion pump operation, sterile technique, and catheter maintenance for home care.
- Collaborate with prescribing physicians, home health nurses, and internal teams including intake, referral services, reimbursement, nursing, clinical services, and pharmacy operations to ensure seamless therapy management and reduce hospital readmissions.
- Maintain regulatory compliance with State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other applicable standards.
- Manage pharmacy inventory, including refrigerated products, and prepare medication shipments for timely delivery to patients’ homes.
Qualifications:
- Minimum of 3 years’ experience in infusion specialty pharmacy.
- IVIg and hemophilia experience are highly preferred.
- Strong knowledge of specialty therapy payer requirements, reimbursement processes, and contract obligations.
- Current and good standing NY pharmacist license.
- Experience with CareTend system preferred.
