Job Description
Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.
A direct hire opportunity is available for a Clinical/Infusion Pharmacist in the Syracuse, NY area. This role involves preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications for patients receiving home infusion therapy. The pharmacist serves as a core member of a multidisciplinary team, ensuring optimal medication management and patient education.
Location: Manlius, NY (Syracuse area)
Schedule: Monday - Friday, 8:00 AM – 5:00 PM
On Call: Participate in on-call rotation (typically fewer than 5 after-hours calls weekly)
Key Responsibilities:
- Prepare specialty pharmaceuticals and IV solutions in a sterile environment in compliance with state and federal regulations.
- Assess laboratory values, perform pharmacokinetic dosing (e.g., vancomycin), and monitor Total Parenteral Nutrition (TPN) therapy.
- Provide detailed patient counseling and education on specialty medications and treatment protocols.
- Train patients and caregivers on infusion pump use, sterile techniques, and catheter maintenance for home care.
- Collaborate closely with physicians, nurses, and clinical teams to coordinate care and reduce hospital readmissions.
- Ensure compliance with regulations including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA.
- Manage pharmacy inventory including refrigerated stock and prepare medications for safe home delivery.
Qualifications:
- Minimum of 3 years’ experience in infusion specialty pharmacy.
- Strong preference for candidates with IVIg and hemophilia experience.
- Knowledgeable in specialty therapy payer requirements, reimbursement procedures, and contractual obligations.
- Familiarity with CareTend system preferred.
- Current and valid New York pharmacist license in good standing.
