Work Environment & Population Served

Our facilities operate as Skilled Nursing Facilities (SNFs), providing:

Position Overview

The Physical Therapist (PT) will deliver direct rehabilitation care in physical therapy to patients referred by physicians, adhering to all relevant laws, regulations, and organizational standards.

Qualifications and Experience
  • Graduation from an accredited physical therapy program (BSPT, MSPT, or DPT; those graduating in 2003 or later must hold an MSPT or DPT)
  • Current registration/licensure in the applicable state, with an active license in good standing throughout employment
  • One year of experience is preferred; however, new graduates are welcome to apply
  • CPR certification required upon hire or to be obtained during orientation, with maintenance of current certification throughout employment
Specific Job Responsibilities
  • Engage in community health initiatives/projects as appropriate
  • Proficient in Microsoft Word, Excel, and email communication
  • Collaborate with patients, families, and support departments to effectively plan for patient needs
  • Exhibit proper body mechanics at all times
  • Make independent decisions when necessary
  • Understand and comply with practices, procedures, laws, regulations, and guidelines governing post-acute care
  • Implement and interpret departmental programs, goals, objectives, policies, and procedures
  • Demonstrate proficiency in all competency areas, including patient rights, safety, and sanitation
  • Maintain professional relationships with all associates and vendors
  • Ensure confidentiality of all proprietary and confidential information
  • Adhere to company policies, including those related to harassment and compliance
  • Exhibit integrity and professionalism by following the organization’s Code of Conduct and completing mandatory compliance training
Essential Functions
  • Implement physical therapy treatment plans for patients
  • Set, assess, and adjust realistic, measurable, timely, and functional goals
  • Supervise and evaluate care provided by PTAs, Rehab Aides, and PT and PTA students
  • Document care accurately and in a timely manner
  • Utilize therapy software effectively and accurately
  • Provide excellent customer service and maintain a positive attitude towards patients
  • Assist in the evacuation of patients when necessary
  • Demonstrate reliable attendance
  • Utilize reasoning skills and sound judgment
  • Communicate and collaborate effectively within an interdisciplinary team
  • Engage in physical activities such as sitting, standing, bending, lifting, pushing, pulling, stooping, walking, reaching, and moving intermittently during working hours
  • Read, write, speak, and comprehend the English language
