Job Description
This PRN Pharmacist position offers the opportunity to provide multi-site coverage within the Cleveland, OH area. As a local hospital pharmacist, you will support several inpatient pharmacy locations, delivering high-quality clinical pharmacy services on an as-needed basis for a 13-week contract. Ideal candidates possess an active Ohio pharmacist license and demonstrate strong skills in inpatient pharmacy practice, clinical medication management, and interdepartmental collaboration.
Responsibilities include:
• Completing medication reconciliation and developing accurate prior-to-admission medication lists
• Providing discharge medication counseling to patients and families
• Monitoring drug therapy for contraindications, interactions, allergies, and appropriate dosage
• Offering clinical consultation and medication recommendations to healthcare providers
• Participating in antimicrobial stewardship activities and interpreting culture results
• Performing pharmacokinetic and anticoagulation dosing and monitoring
• Supporting Pharmacy & Therapeutics Committee functions and quality assurance initiatives
• Maintaining medication inventory and ensuring timely procurement and accurate dispensing
• Verifying medication orders and maintaining controlled substance accountability
• Managing emergency medications, medication storage, and compliance with policies
• Educating pharmacy staff, healthcare professionals, and patients with medication information
• Following facility safety protocols to ensure a secure patient-care environment
Qualifications and desired experience:
• Active Ohio Pharmacist license required
• Hospital/inpatient pharmacy experience strongly preferred
• PRN availability with day shift flexibility
• Willingness to travel between Cleveland-area facilities as needed
• EPIC electronic health record experience preferred
• Strong clinical, communication, and independent working skills
• Experience with medication reconciliation, antimicrobial stewardship, pharmacokinetics, and anticoagulation procedures
