An experienced pharmacist is needed to deliver comprehensive pharmacy services at a 35-bed long-term acute care hospital specializing in critical illness recovery. This role involves managing medication distribution, sterile product preparation, medication reconciliation, clinical pharmacy consultation, and ensuring compliance with regulatory standards. The position requires a pharmacist licensed to practice and skilled in inpatient hospital pharmacy, with demonstrated ability in EPIC electronic medical record usage.

Location: Bettendorf, IA (hospital based on the 3rd floor of a medical center in Davenport, IA)
Schedule: Days, 8-hour shifts, 5 days per week
Contract Length: 13 weeks
Start Date: ASAP

Key Responsibilities:

  • Perform thorough medication reconciliation and prepare accurate home medication lists for patient admissions.
  • Provide discharge medication counseling to patients and their families to ensure proper understanding of therapy.
  • Monitor drug therapies for interactions, allergies, and dosing accuracy, offering clinical consultations to healthcare providers.
  • Participate in antimicrobial stewardship, including interpreting culture results and making infection-related recommendations.
  • Conduct pharmacokinetic and anticoagulation dosing and monitoring to optimize patient outcomes.
  • Manage pharmacy operations including drug inventory, sterile preparation, medication order verification, and compliance with controlled substance regulations.
  • Maintain pharmacy records, handle medication returns and recalls, and perform environmental and safety inspections.
  • Engage in staff training, provide educational resources, and serve as a resource for pharmacy personnel and multidisciplinary teams.
  • Uphold strict adherence to safety protocols and regulatory compliance in all pharmacy activities.

Desired Experience and Qualifications:

  • Active pharmacist license valid for the assignment location
  • Hospital and long-term care pharmacy experience strongly preferred
  • Proficient in EPIC electronic medical records system
  • Strong clinical judgment and effective communication skills
  • Experience with medication reconciliation, antimicrobial stewardship, pharmacokinetic dosing, and anticoagulation
  • Ability to work independently in a fast-paced hospital/LTACH setting
