Join Our Team as a Family Medicine Physician

We are excited to invite a dedicated and enthusiastic Family Medicine Physician to become a part of our Primary Care & Pediatrics team in Rockford. This role presents a unique chance to significantly influence the health of our community while thriving in a supportive, mission-oriented setting.

Position Details

• Employment Type: Full-time (1.0 FTE)
• Office Hours: Monday to Friday, 8 AM to 5 PM
• Practice Type: Outpatient Family Medicine
• Team Composition: 8 Family Medicine Physicians, 2 Pediatricians, 1 Family Medicine Nurse Practitioner, and 2 Pediatric Nurse Practitioners
• Clinical Care Team: 2 providers and 3 Medical Assistants/LPNs
• Teaching Opportunities: Available for Residents, Medical Students, and Fellows
• On-call Coverage: 1 week every 2 months, from 5 PM to 12 AM on weekdays and 7 AM to 7 PM on weekends
• Patient Panel: Ready to develop immediately

Key Responsibilities

• Provide high-quality, comprehensive care in a collaborative, patient-focused environment.
• Utilize advanced digital tools such as Epic and MyChart for efficient patient management.
• Collaborate with skilled physicians, Advanced Practice Providers (APPs), and committed support staff.
• Engage in quality improvement initiatives and support community health efforts.
• Document patient interactions in the Epic Electronic Medical Record (EMR) system.

Qualifications

• Active Michigan medical license or the ability to obtain one.
• Open to recent graduates.
