This contract role offers hands-on experience working one-on-one with a fifth-grade student who requires specialized medical care throughout the school day.

Key Qualifications:

• Current and active LPN license in Georgia
• Prior experience providing individualized care, preferably with school-aged children in a school-based setting
• Comfortable with g-tube feeding, occasional supplemental oxygen, and vital sign monitoring
• Ability to facilitate daily transportation care by accompanying the student from home to school and back, as per IEP and medical directives
• Compassionate, attentive, and able to follow clear care protocols

Duties and Responsibilities:

• Deliver daily health services to a 5th-grade student, including g-tube management, vital sign monitoring, and oxygen supplementation as needed
• Support the student during transportation by meeting at their home, ensuring safe care during bus transit, and accompanying them safely back in the afternoon
• Coordinate with educational staff and the student’s family to ensure seamless health management and compliance with physician’s orders and the IEP
• Maintain accurate and timely health records and communication with care team
