An experienced pharmacist is needed to provide comprehensive pharmacy services in a 35-bed Long-Term Acute Care Hospital (LTACH) specializing in critical illness recovery. This role focuses on supporting medication distribution, sterile product preparation, clinical pharmacy services, medication reconciliation, patient counseling, and regulatory compliance within a hospital setting utilizing EPIC electronic medical records.

Qualifications / Desired Experience:

  • Active pharmacist license appropriate for hospital assignment
  • Previous experience in hospital or inpatient pharmacy environments preferred
  • Proficiency with EPIC electronic medical records system
  • Strong clinical knowledge and excellent communication skills
  • Ability to work independently in an LTACH/hospital environment
  • Expertise in medication reconciliation and preparation of discharge medication lists
  • Experience with antimicrobial stewardship protocols
  • Skills in pharmacokinetic and anticoagulation dosing and monitoring

Location:

  • Eldridge, IA (hospital located inside a medical center on the 3rd floor)

Responsibilities:

  • Complete thorough medication reconciliation and develop accurate home medication histories at admission
  • Provide discharge medication counseling to patients and families
  • Monitor drug therapy for interactions, allergies, contraindications, and appropriate dosing
  • Offer clinical consultation and medication recommendations to medical staff
  • Participate in antimicrobial stewardship including culture analysis and infection treatment recommendations
  • Conduct pharmacokinetic and anticoagulation dosing and monitoring
  • Maintain medication inventory, preparation, verification, and compliance with regulatory standards
  • Manage controlled substances, emergency medications, medication returns, and documentation
  • Support pharmacy operations including staff education, quality assurance, and committee participation
  • Adhere to safety and compliance policies to ensure a safe patient care environment

Schedule:

  • Day shifts
  • 8-hour shifts, 5 days per week

This is a 13-week contract position with an immediate start. Candidates with hospital pharmacy experience, EPIC system knowledge, and strong clinical capabilities in the LTACH environment will excel in this role.
