Home Infusion Clinical Pharmacist - Westerville, Ohio

Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

This direct hire opportunity seeks a Clinical/Infusion Pharmacist dedicated to preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications in a home infusion setting. Located in Westerville, OH, this role supports patients by acting as a key clinical resource within a multidisciplinary team, ensuring safe and effective therapy management.

Schedule/Hours:

• Monday to Friday, 8:00 AM – 4:30 PM
• Participate in on-call rotation

Key Responsibilities:

• Prepare specialty pharmaceuticals and sterile IV solutions in compliance with state and federal regulations.
• Assess lab values, perform pharmacokinetic dosing calculations (e.g., vancomycin), and monitor Total Parenteral Nutrition (TPN) therapy.
• Provide patient counseling and education on specialty medications and infusion techniques.
• Train patients and caregivers on using infusion pumps, sterile methods, and catheter care at home.
• Collaborate with physicians, nurses, and pharmacy teams to coordinate care and prevent hospital readmissions.
• Maintain compliance with regulatory standards, including HIPAA, DEA, Medicare, ACHC, URAC, OSHA, and State Board of Pharmacy guidelines.
• Manage inventory, including refrigerated products, and ensure timely, safe delivery of medications to patients.

Qualifications and Experience:

• Minimum of 2 years of infusion specialty pharmacy experience.
• IVIg experience is a plus.
• Knowledge of specialty therapy payer requirements, reimbursement processes, and contract obligations.
• Current and active Ohio pharmacist license in good standing.
• Experience with CareTend system preferred.
