Hospice Home Care Visit Nurse RN

Position Overview

The role centers on providing compassionate, highquality hospice nursing care to patients in their homes or facilities. This contract is ideal for an RN who is confident in endoflife care, comfortable with autonomy, and skilled in supporting patients and families through complex emotional and clinical needs.

Key Responsibilities

• Deliver comprehensive hospice nursing care, including assessments, symptom management, and care planning
\n
• Coordinate with interdisciplinary team members (social workers, chaplains, aides, physicians)
\n
• Educate patients and families on disease progression, comfort measures, and available resources
\n
• Maintain accurate, timely clinical documentation
\n
• Participate in case conferences and careplanning meetings
\n
• Provide oncall support if required (specify if applicable)
\n

Required Qualifications

\n
• Active RN license in the state of practice
\n
• Minimum 1 year of hospice nursing experience (more preferred)
\n
• Strong clinical assessment and criticalthinking skills
\n
• Ability to work independently in the field
\n
• Reliable transportation and valid driver’s license
\n

Hourly plus Mileage at .72 per mile
