Job Description
Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.
A clinical pharmacist specializing in home infusion pharmacy plays a pivotal role in preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications for patients in a home care setting. This position requires a strong clinical focus and the ability to collaborate effectively with a multidisciplinary healthcare team to ensure optimal patient outcomes.

Responsibilities:
- Sterile Compounding: Prepare specialty pharmaceuticals and IV solutions in a sterile, ventilated environment compliant with state and federal regulations.
- Clinical Monitoring: Evaluate laboratory values, calculate pharmacokinetic dosing (such as vancomycin), and monitor Total Parenteral Nutrition (TPN) therapies. Provide patient counseling and education on specialty medications and treatment plans.
- Patient Onboarding & Education: Train patients and caregivers on infusion pump use, sterile techniques, and catheter maintenance at home.
- Care Coordination: Work collaboratively with prescribing physicians, home health nurses, and various internal teams including intake, reimbursement, nursing, clinical services, and pharmacy operations to ensure smooth therapy management and reduce hospital readmissions.
- Regulatory Compliance: Ensure adherence to State Board of Pharmacy regulations, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other applicable standards.
- Inventory & Shipping: Manage pharmacy inventory including refrigerated products and oversee the safe preparation and shipping of medications and supplies to patients’ homes.
Qualifications:
- Minimum of 2 years’ experience in infusion specialty pharmacy.
- IVIg experience is highly desirable.
- Familiarity with specialty therapy payer requirements, reimbursement processes, and contract obligations.
- Current and active pharmacist license in New York in good standing.
- Experience using CareTend system preferred.
