Position OverviewAs a vital member of our interdisciplinary healthcare team, you will be responsible for delivering medical nutrition therapy. You will ensure continuity in the Nutrition Care Process throughout all stages of patient care, following guidelines set by the Academy of Nutrition and Dietetics. Your role will involve conducting nutrition assessments for patients identified as being at nutritional risk, based on established criteria and best practices. You will calculate metabolic nutrient needs tailored to the specific age and health conditions of patients, perform nutrition-focused physical examinations to assess malnutrition risk, and develop appropriate nutrition diagnoses aligned with the Nutrition Care Process.
Your responsibilities will include creating and implementing nutrition intervention and care plans for patients with identified nutrition diagnoses, continuously evaluating the effectiveness of these interventions, and providing nutrition education to patients, families, and healthcare team members. Topics may encompass specialized diets, enteral/oral medical nutrition supplements, food choices, eating behaviors, and community nutrition resources. You will manage the delivery of nutrition care services based on your assigned care site and engage in performance improvement and research initiatives. Additionally, you will mentor dietetic interns and technicians while actively assessing and enhancing your professional development and competencies as outlined in your role.
Key Responsibilities- Deliver medical nutrition therapy in accordance with organizational policies and A.N.D. Nutrition Care Process standards.
- Conduct nutrition screenings, assessments, diagnoses, interventions, and evaluations for at-risk patients.
- Utilize evidence-based, age-appropriate nutrition care, considering psycho-social factors.
- Document all patient interactions in the healthcare record following hospital and departmental guidelines.
- Monitor and evaluate enteral nutrition support regimens, managing them under medical supervision.
- Develop and implement transitional feeding plans.
- Perform nutrition-focused physical exams to assess malnutrition severity as needed.
- Evaluate patients' learning needs and barriers, tailoring nutrition education accordingly.
- Leverage community resources to enhance patient outcomes.
- Collaborate with patients, families, and the healthcare team to assess nutrition and education needs.
- Establish goals and provide services based on the identified needs of patients and families.
- Continuously assess nutrition care processes and patient outcomes, implementing quality practices based on system policies and evidence-based research.
- Adhere to organizational ethics, licensure, and professional standards in dietetics.
- Participate in multidisciplinary rounds, committees, and team meetings.
- Utilize best practice research to enhance disease management and patient outcomes.
- Communicate scientific principles and research findings to team members and interns.
- Document interactions and share knowledge with interns, patients, and healthcare team members.
- Engage in quality improvement initiatives and projects.
- Conduct self-assessments to identify professional strengths and areas for growth.
- Participate in continuous learning opportunities to enhance skills and competencies.
- Perform additional duties as assigned, adhering to all organizational policies and standards.
- Bachelor's degree in Dietetics or Nutrition.
- Certification in Dietetics or Nutrition is also acceptable.
All certifications must be obtained upon hire unless specified otherwise:
- BLS - Basic Life Support or equivalent certification within 30 days.
- LD - Licensed Dietitian or provisional license within 180 days.
- RD - Registered Dietitian or Registered Dietitian Nutritionist, or provisional status within 180 days.
A minimum of one to two years of clinical experience is required. Preference will be given to candidates who have completed a supervised practice program accredited by the Accreditation Council for Education in Nutrition and Dietetics (ACEND).
