A healthcare facility specializing in long-term acute care seeks a PRN Pharmacist to support medication management and clinical pharmacy services on an as-needed basis. This role involves providing expert pharmaceutical care in a 34-bed LTACH/critical illness recovery setting, ensuring optimal patient outcomes through comprehensive medication reconciliation, drug therapy monitoring, and patient counseling.

Key responsibilities include:

  • Completing thorough medication reconciliation to develop accurate home medication lists prior to admission.
  • Offering discharge medication counseling to patients and family members.
  • Monitoring drug therapies for contraindications, interactions, allergies, and dosage appropriateness.
  • Providing clinical consultation and medication recommendations to the healthcare team.
  • Participating in antimicrobial stewardship initiatives, including culture interpretation and therapy guidance for infections.
  • Performing pharmacokinetic and anticoagulation dosing and monitoring.
  • Managing medication inventory, sterile product preparation, and verification processes.
  • Ensuring compliance with pharmacy laws, regulations, and accreditation standards.
  • Contributing to staff education, orientation, and promoting a culture of safety and quality improvement.

Qualifications and experience:

  • Active pharmacist license applicable to the assignment.
  • Hospital or inpatient pharmacy experience preferred.
  • Proficiency with EPIC electronic health records.
  • Strong clinical knowledge and communication skills.
  • Ability to work independently in an LTACH environment.
  • Availability for PRN/as-needed daytime shifts.
  • Experience in medication reconciliation and discharge medication lists.
  • Background in antimicrobial stewardship, pharmacokinetic, and anticoagulation dosing/monitoring.
