Job Description
Our Client, a Greater Manchester council is seeking an Adult’s Social Worker to join either one of their Neighbourhood, Transitions or LD Team.

\nResponsibilities of this role:
\nLearning Disabilities (LD) Team
\n- \n
- Undertake assessments, reviews, and support planning for adults with learning disabilities, promoting independence and person-centred outcomes. \n
- Manage safeguarding, risk, and Mental Capacity Act responsibilities in line with legislation and best practice. \n
- Work collaboratively with families, health professionals, and partner agencies to coordinate effective support and achieve positive outcomes \n
Transitions Team
\n- \n
- Support young people transitioning from Children's Services to Adult Social Care through assessment, planning, and review processes. \n
- Promote independence and positive outcomes by working closely with young people, families, education, health, and partner agencies. \n
- Manage safeguarding, risk assessment, and statutory responsibilities in line with relevant legislation and guidance. \n
Neighbourhood Team
\n- \n
- Complete Care Act assessments, reviews, and support planning for adults with a range of care and support needs. \n
- Manage safeguarding enquiries, risk assessments, and Mental Capacity Act duties to ensure individuals' safety and wellbeing. \n
- Work in partnership with residents, carers, and multidisciplinary teams to deliver strengths-based support and positive outcomes. \n
Requirements of this role:
\n- \n
- Must be a Qualified Social Worker, registered with Social Work England. \n
- Must have minimum of 3 years post qualifying social work experience \n
