Ambulatory Care Pharmacist job in Portland, OR
Health System setting.
Monday - Friday, day-shift position
Ambulatory Care Pharmacy duties:
- Direct care of patients
- Education of physicians and staff regarding clinical information
- Implementation of clinical programs that involve patients/ drug therapy.
- Responsible for managing the selection and utilization of pharmaceuticals and supports core clinical programs such as DUR, DIS and formulary management.

Ideal candidate: BS in Pharmacy or PharmD. Prior Ambulatory Care experience or PGYI. Active OR pharmacist license or ability to obtain.
