Join our team as a Home Health Registered Nurse (RN) and provide compassionate, patient-centered care in the comfort of patients' homes. Home Health Nurses play a critical role in helping patients manage chronic conditions, recover from illness or surgery, and maintain independence while receiving skilled nursing services outside of a traditional healthcare setting. This role offers the opportunity to build meaningful patient relationships while delivering individualized care plans that improve outcomes and quality of life.
Position Details

- Job Type: Block Booking / Per Diem Opportunities Available
- Shift: 12-Hour Shifts
- 7:00 AM – 7:00 PM
- 7:00 PM – 7:00 AM
- Location: Home Health Setting (Vary per assignment)
- Start Date: ASAP
Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans.
- Provide skilled nursing care, treatments, and medication administration.
- Monitor patient progress and communicate changes in condition to physicians and care team members.
- Educate patients and family members on disease management, medications, and home care techniques.
- Maintain accurate and timely clinical documentation.
- Coordinate care with physicians, therapists, case managers, and caregivers.
- Ensure compliance with all agency, regulatory, and patient safety standards.
Qualifications
- Active Registered Nurse (RN) license in the applicable state.
- Home Health experience preferred.
- Current BLS certification required.
- Valid driver's license and reliable transportation.
- Strong assessment, communication, and critical-thinking skills.
- Ability to work independently and manage patient caseload effectively.
